Labeling Boxes and Devices

This section includes the following:

Kit Number Label

Vivify Health provides you with the template for this label, including the bar code font. Do the following:

  1. Purchase the blank labels and print them:  SL105 1 3/4” x 1/2” for the kit # next to the box handle. You can purchase the labels on the Uline website.
  2. Place the label on the box.

Hospital Label

Vivify Health provides you with the template for this label, including the graphic.

  1. Purchase the following blank labels and print them: Uline 4 X 6 Laser labels for the hospital graphic on the outside of the box. You can purchase the labels on the Uline website.

    Label specification: Part #: S-10423 Uline 4 X 6

  2. Place the label on the box.

Device Labels

  1. Order small labels p/n ZA 4349 from Anchor Graphics Marketing via phone: 800-875-7859 and order the green dot stickers via Amazon.
  2. Place the label on the devices. For more information see Label Devices.

    Procedure

    		Example
    TabletPlace a kit label sticker on the back, centered near the bottom. Place the green dot sticker on the front near the power button.

    Weight ScalePlace a kit label sticker on the back, centered above the model plate. 
    Blood PressurePlace a kit label sticker on the back of the device.
    OximeterPlace a kit label sticker to the side opposite of where the finger is inserted. 
    GMAPlace a kit label sticker on the side of the GMA. 
    ThermometerPlace a kit label sticker on the right side of the device.
